Most failures report themselves the moment they happen. The one exception is the whole execution dying before it ever gets the chance to report anything at all. Xara watches for that on its own: a task that goes quiet for too long is marked stalled and moved to Needs Attention, so it never sits invisibly stuck with nothing actually happening.
